2025 Award Application Instructions
All Surgical VR Fellows training in the United States, whose second year of training will commence in July 2025, are eligible to apply.
The following materials are due by July 11:
- Online Application Form
- Curriculum Vitae
- Personal Statement
- Photo
- 5 letters of recommendation
- Retina Fellowship Program Director
- Residency Program Director
- 3 individuals familiar with your training and/or research activities
Application Process
Applicants must set up a password-protected account log-in.
Applicants are responsible for completing the online application form, uploading a CV, personal statement, and photo. Applicants will be able to request their five letters of recommendation through the application system.
Before finalizing the application, applicants will be able to replace any documents and/or their photo before the deadline. Once the deadline has passed, any revisions to applications will be made by the Michels Foundation Administrator.
Once applications are complete, applicants will receive an email notification.
